TEVA (Transition from School to Active Life)

TEVA (Transition from School to Active Life) is a collaborative planning process that helps students prepare for life after high school. TEVA brings together the student, their family, the school team, professionals, and community partners to support a successful transition to adulthood. The student’s interests, strengths, needs, and future goals guide the planning process every step of the way. The goal is to help students build a meaningful life plan and connect with the services, support, and opportunities they may need after leaving school.

Who TEVA Supports

TEVA may be appropriate for high school students who:

• Are between 15 and 21 years old

• Have a disability such as autism, an intellectual disability, a mental health condition, or another limitation that impacts their independence

• Have an Individualized Education Plan (IEP)

• Would benefit from coordinated planning and support as they prepare for adult life

The Student Life Plan

TEVA helps students develop a personalized life plan based on the areas of life that are important to them. Together, the student and their support team identify goals and plan the steps needed to achieve them. The Student Life Plan is reviewed and updated regularly to reflect the student’s evolving goals, strengths, interests, and needs. Life Plan

The TEVA Process

Planning starts several years before a student intends to leave high school and is initiated by the school team.

  • Student – Shares their goals, strengths, interests, and dreams for the future, and is willing to learn, participate, and try new experiences.
  • Parents, Guardians, and Family Members – Provide insight, encouragement, and support throughout the transition process.
  • School TEVA Team (Administrators, Teachers, and Support Staff) – Coordinates transition planning and provides learning opportunities, experiences, and supports that help students prepare for adult life.
  • Complementary Services Professionals -Collaborate with school teams by sharing professional insights, strategies, and recommendations that support successful transitions.
  • Community Partners– Connect students and families with services, resources, supports, and opportunities in their community that support life after school.

Starting early helps prevent gaps in services and increases the likelihood that students will have the support, activities, and opportunities they need when they transition to adult life.
