Information on the Vaccination of Youth Aged 12+

THIS PAGE IS ARCHIVED
It does not reflect the directives and measures in place for the 2021-2022 school year

As you may be aware, vaccination of youth aged 12 and above has been approved and is underway in our communities. School boards have been tasked with assisting with registration and access for students in this age group. Vaccination remains voluntary and our schools will support access to this service.

CISSSO is finalizing the preparations for the COVID-19 vaccinations for students aged 12 years and above.  All students wishing to receive the vaccination through the school, must pre-register through Clic-Santé before the day of the vaccination.  Adult Education students must notify administration if they are interested in pursuing the vaccination through the school process.  Students may register themselves or be registered by a parent.  If this is not completed the school may be able to assist the student with registering themselves at the school level.  Even if your child is receiving their vaccination from a mobile site (at your school or another location), you must pre-register.

When entering Clic-Santé’s portal (links below), you must select your child’s school and complete the required sections:

  • name and family name of the student (as it appears on the health insurance card)
  • name and family name of mother
  • date of birth
  • health card number
  • email address (to obtain the vaccination passport)
  • phone number

The time of your appointment has no importance, however if your school has two scheduled dates (this is applicable for Hadley/PWHS and Symmes/D’Arcy only), it is important to indicate the appropriate date for your group/class.  Please consult your school information as to which date is yours.

If you wish to obtain the vaccine through the school supported process, please pre-register at:

  • If your child is 12-13, you must also complete and return (to the school) your signed consent form
  • Students aged 14 and above will provide their individual consent at the vaccination site
  • It is essential that your child takes their medical card with them on the assigned day of the vaccination

Once again, students who are currently accessing virtual learning platforms, may access vaccination within their respective community during the same time period and you may continue to access vaccination within your community at your discretion, and can do so through Clic-Santé.

Measures will be taken to ensure a safe and secure coordination and travel to and from the vaccination sites.  Students will be organized and travel by/with their class groupings, distancing between groupings will be maintained, students will travel on the same bus to and from the site, attendance will be taken before departing from the school and vaccination site, teachers/supervisors will accompany the students, and students will wear procedural mask and disinfect their hands prior to boarding the bus.  Once at the vaccination site, the vaccination team and supervisors will ensure a safe process for our students.

Please consult the MSSS documents for more information on the vaccine and consent:
